Bilingual Case Management

Older adults and family members often find themselves navigating through a maze of care options. J-Sei’s bilingual case managers can help identify solutions that help promote independence and well- being. Case managers conduct a home visit and assess the older adult’s abilities, health, home safety and social supports. This assistance is helpful both in a crisis and for long term planning situations.

Case managers work with families to secure the necessary help and resources such as:

  • Interpreter services for medical appointments
  • In-home care
  • Transportation
  • Re-location to a new living arrangement
  • Legal and financial services
  • Information and referrals
  • Minor home repairs
  • Nutrition programs
  • Friendly Visitors/Caller referrals

ELIGIBILITY
Must be at least 60 years of age.

DONATION/FEE FOR SERVICE
Families are asked to contribute towards the cost of service on a sliding scale basis.

Transportation

Our Transportation Program began in March of 2010 as a way to bring older adults who no longer feel comfortable driving themselves to the J-Sei Senior Center. This program was designed to address two needs that were becoming more apparent in the community: poor nutrition and lack of social options for people who have given up driving. Transportation service is available Monday, Tuesday, and Thursday to bring seniors to the J-Sei Senior Center for lunch, to a local market for grocery shopping, and even to certain J-Sei exercise classes. On Wednesdays, the van takes riders to a fun destination for shopping or sightseeing.

ELIGIBILITY

  • Must be at least 60 years of age.
  • Must be able to walk with minimal assistance and sit in van for a maximum of one hour.
  • Each attendee must complete an updated registration form each fiscal year.
  • At this time, our van does not have wheelchair capabilities.

FEE FOR SERVICE
An $8.00 roundtrip fee to the Senior Center is requested for the van service. Costs of rides to other destinations vary upon location.

Friendly Visitors & Callers

As we age, keeping up old friendships or making new acquaintances may grow more difficult. Yet having a friend to laugh with or someone to chat with about current events or sports is so important to our health and happiness. J-Sei’s volunteer visitors and callers are matched with an older adult who would benefit from lively conversation and companionship. Trained and screened volunteers make weekly connections through in-person visits or phone calls.

ELIGIBILITY

  • Must be at least 60 years of age.
  • Have difficulty with daily living tasks such as preparing meals, shopping, money management, using the telephone or doing light housework.

DONATION/FEE FOR SERVICE
Families may provide a donation if possible.

LOCATION
Friendly Visitors visit their companion in his/her own home. Friendly Callers call from their own home.

Caregiver Directory

In-home assistance for a variety of tasks such as house cleaning, personal care, or shopping is available through the J-Sei Caregiver Directory. This is a listing of experienced in-home care workers who are culturally sensitive and/or bilingual. The Case Managers use the Caregiver Directory to match families with suitable in-home care workers. J-Sei screens potential in-home care workers but the family interviews, hires, supervises and pays the in-home care worker as they are not employed by J-Sei.

  • Complete and submit the J-Sei application
  • Interview with a J-Sei Program Coordinator
  • Pay J-Sei a $15 background check fee
  • Provide references
